The IU Methodist Family Medicine Center is the department’s largest out-patient site and is home to the IU Family Medicine Residency Program. The Center has approximately 25,000 square feet, 30 exam rooms, two family counseling rooms, procedure rooms, an onsite lab and an X-ray facility. The FMC celebrated the 10th anniversary in the fall of 2008.
Patient Centered: The heart of the IU Methodist Family Medicine Center is a physician-patient relationship that is highly satisfying for both the physician and the patient. Patients are encouraged to take an active part in their care.
Whole-Person Orientation: IU Methodist Family Medicine Center is designed to provide broad range of complete health care services. The physicians are primary care providers for each patient's health care; making use of the experience and expertise of specialists at Clarian Heath Partners at Methodist Hospital.
Health Information Technology: IU Methodist Family Medicine Center uses an "advanced information system" to deliver and improve your heath care. Electronic health records (EHR) help communication with patients and other health care providers. EHR's also help physician care and management of chronic diseases.
Continuity of Care: The physicians at IU Methodist Family Medicine Center will help prevent serious illness. They are trusted physicians that will treat and manage all aspect of illness or injury if it affects your family. They are specialists who coordinate your care in a complex health care system.

Welcome Letter
Dear Anticoagulation Clinic Patient:
Welcome to the IU Methodist Family Medicine Center Anticoagulation
Clinic! Your physician has referred you to this clinic for the
management of your Coumadin® (warfarin) therapy. The purpose of this
letter is to provide you with general information about our clinic and
to let you know what to expect on your first visit.
This is a clinical pharmacist-managed clinic that is overseen by a physician Medical Director. The overall goal of this clinic is to improve the care of patients receiving anticoagulation therapy. The purpose of the clinic is to provide uniform and continuous monitoring with appropriate dose adjustment, patient education, and coordination of laboratory tests for patients placed on Coumadin® (warfarin). Essentially, our role is to ensure that your Coumadin® (warfarin) works well for you. We do this by educating you about Coumadin® (warfarin), by closely monitoring your level of anticoagulation, and by ultimately decreasing adverse reactions associated with this therapy.
At the IU Methodist Family Medicine Center Anticoagulation Clinic, we will see patients by appointment on Tuesday afternoons between 1:00 pm & 5:00 pm. Anticoagulation Clinic appointments may be scheduled through the scheduling center at 962-1041. The center is located directly across the street from the Methodist Hospital Emergency Department. We ask that you bring all of your medications to your first clinic visit. We also ask that you bring your insurance card with you.
At each visit we will draw your blood in the Family Medicine Center lab area. Ideally, you should not have to wait more than a few minutes to have the blood drawn.
Your first appointment should last approximately one hour. We estimate that subsequent appointments will last approximately 30 minutes. We look forward to seeing you for your first visit!

Brief Guide to Coumadin® (warfarin) Anticoagulation Therapy
Coumadin® (warfarin) has been ordered for you by your doctor to take at home. This medication is an anticoagulant used to prevent harmful blood clots from forming in the veins, arteries, and heart. Harmful blood clots can cause a stroke ("brain attack") or a heart attack, among other things.
It is important that patients have regular laboratory tests
performed. The primary test used during anticoagulation therapy is the
prothrombin time (Pro-Time) test. This test is best reported as the INR.
The INR allows the doctor and pharmacist to monitor the patient's
Coumadin® (warfarin) therapy and make dosage adjustments if necessary.
While taking Coumadin® (warfarin) it is very important that you:
- Always tell other doctors, dentists, pharmacists, or nurses that you are on Coumadin® (warfarin).
- Always have regular blood tests (PT-INR) to check how quickly your blood is clotting.
- Discuss all medications (prescription, over-the-counter, herbal) with your doctor and pharmacist. Many drugs interact with Coumadin® (warfarin).
- Keep your eating habits and activities similar every day.
- NEVER "double-up" on a dose to make up for a missed dose. Keep a record of the missed dose and report it to your doctor or pharmacist at the next visit. Call your doctor if more than one dose is missed.
- Do not take any aspirin or aspirin-containing products. Be sure to read labels carefully or ask your pharmacist if there is any doubt.
- Avoid excessive amounts of alcohol. The anticoagulant effect can be increased or decreased by the intake of alcohol.
- Keep Coumadin® (warfarin) out of the reach of children and pets.
- Do not take Coumadin® (warfarin) if you are pregnant or plan to become pregnant.
- Notify your doctor and pharmacist if other medications are started or discontinued, or if any dosages are changed.
- Always carry your identification card with the correct name and dosage of your anticoagulant.
The most common side effect associated with Coumadin® (warfarin) therapy is bleeding. Watch for the following warning signs or events and call your doctor immediately if you experience any of the following:
- A serious fall or hard bump to the head
- Bleeding that does not stop, either from a cut or from your nose
- More bleeding than usual when brushing your teeth
- More bleeding than usual during your menstrual period or abnormal bleeding from the vagina
- A dark brown or reddish discoloration of your urine
- Bloody, black, or tarry stools
- Unusual bruising
- Throwing up (vomiting) or coughing up blood
- Fever, diarrhea, or infection
- Pain or swelling in your feet or legs
- Dizziness, trouble breathing, chest pain, weakness or fatigue
- Numbness, confusion, unsteadiness when walking
