Welcome Letter to Coumadin^® (warfarin) Anticoagulation Therapy

Dear Anticoagulation Clinic Patient:

Welcome to the Indiana University • Methodist Family Practice Center Anticoagulation Clinic!  Your physician has referred you to this clinic for the management of your Coumadin^® (warfarin) therapy.  The purpose of this letter is to provide you with general information about our clinic and to let you know what to expect on your first visit.

This is a clinical pharmacist-managed clinic that is overseen by a physician Medical Director.  The overall goal of this clinic is to improve the care of patients receiving anticoagulation therapy.  The purpose of the clinic is to provide uniform and continuous monitoring with appropriate dose adjustment, patient education, and coordination of laboratory tests for patients placed on Coumadin^® (warfarin).  Essentially, our role is to ensure that your Coumadin^® (warfarin) works well for you.  We do this by educating you about Coumadin^® (warfarin), by closely monitoring your level of anticoagulation, and by ultimately decreasing adverse reactions associated with this therapy.

At the IU • Methodist Family Practice Center Anticoagulation Clinic, we will see patients by appointment on Tuesday afternoons between 1:00 pm & 5:00 pm.  Anticoagulation Clinic appointments may be scheduled through the scheduling center at 962-1041.  The center is located directly across the street from the Methodist Hospital Emergency Department.  We ask that you bring all of your medications to your first clinic visit.   We also ask that you bring your insurance card with you.

At each visit we will draw your blood in the Family Practice Center lab area. Ideally, you should not have to wait more than a few minutes to have the blood drawn.

Your first appointment should last approximately one hour.  We estimate that subsequent appointments will last approximately 30 minutes.  We look forward to seeing you for your first visit!
