Application Process
In order to apply for a Sports Medicine Fellowship, please follow these directions:  

1. Go to the American Medical Society for Sports Medicine website at www.amssm.org
2. Click on "Fellowships in Sports Medicine"
3. Click on "Application for Sports Medicine Fellowship"
4. If password window appears, please click "Cancel"
5. The application will appear in Word
