Fellowship programs require recent completion of an accredited residency training program and board certification in family practice. Fellows are employed by Indiana University School of Medicine. Benefit structure, salary, and appointment type differ by fellowship. All applications require: 

• Current Curriculum Vitae
• Personal Statement of Interest
• Medical School Transcript
• Three Current Letters of Reference
• Graduation Certificate or Letter from Program Director of Residency
